New Patients

How to Register as a New Patient

Please call at the surgery, with your NHS number. You will be asked to fill in a registration form and a new patient questionnaire.

The Practice does not unfairly discriminate against patients on the grounds of age, gender, disability, ethnicity, race, religion or sexual orientation.

Please bring a list of your current repeat prescriptions. Before you leave your old surgery, please make sure you have a minimum of 14 days supply of any medications to continue taking until after your registration with us.

If you have complex medical needs, please also ask your previous surgery for a ‘patient care summary’.

You will also be offered a New Patient Registration appointment with the practice nurse.

Your medical records will possibly take time to arrive from your previous Doctor, especially if you are moving into Wales from England or Scotland. 

Temporary Residents 

Temporary residents staying / living within the practice catchment area, are able to register at the practice. Please call in at reception for a Temporary Resident registration form.

Test Results

Test results take time to reach us, so please do not ring before you have been asked to do so. Please phone between 2pm-4pm for test results. Our receptionists are trained to give you your test results and tell you what further action the Doctor has asked for.

Please telephone the relevant hospital for tests requested by the hospital, and not the surgery.

Test results will not be given to third parties, so please telephone or call in for your own test results. Your consent is required before we can divulge them to family members etc.

Change Of Details

If you change your name, status, address or telephone/mobile number please inform reception as soon as possible. It is vital that we have correct contact details for you.

If you move out of the surgery’s catchment area, you will be required to register at a practice closer to your new address within 6 weeks.

For urgent issues please telephone the surgery.

While it may be possible to deal with queries sent via email, consultations with the GP or nurse will not be entered into and you will be asked to make an appointment with the GP or other health care professional.
